You Have a Right to Select Your Health Care Provider!
If your doctor decides you require skilled nursing care or inpatient rehabilitation, you have the freedom to choose among a list of participating Medicare-certified facilities. Your choice should be honored by your doctor, hospital discharge planner, or other referring agency. Although you have a say in which facility you select, your choices may be limited by bed availability or by your insurance coverage. When selecting a provider, there are several factors to consider.
If possible, visit facilities before you are admitted.
At LIFE Center Specialty Hospital, we will be happy to introduce you to your nursing and rehab team. You will also learn during your tour the specific services offered, the philosophy and goals of the staff, and the expectations of the patient and family.
Patients admitted to the LIFE Center Specialty Hospital Skilled Nursing Unit will be encouraged to maximize their potential by building their strength, if needed, and receiving at least one hour of daily rehab. This will enable patients a little extra needed time to prepare for more intensive therapy offered in the inpatient rehabilitation wing of the hospital. LIFE Center offers 33 licensed skilled nursing beds, all of which are private rooms.
Those ready for inpatient rehabilitation will receive three hours of very intensive therapy daily in order to be readied to return home within 10-11 days to resume independent living. While the prior discharge-to-home rate exceeded 80% from the rehab unit back to home, we hope to set even higher standards in order to achieve goals exceeding 90%! LIFE Center offers 26 licensed inpatient rehabilitation beds, all of which are private rooms.
