Admission Criteria
For Inpatent Rehabilitation

A patient in need of inpatient rehabilitation must meet the following admission criteria:
- REQUIRES 24-HOUR PHYSICIAN AVAILABILITY AND
- 24 HOUR REHABILITATION NURSING
- MAINTAINS ABILITY TO TOLERATE A MINIMUM
- OF 3 HOURS OF THERAPY PER DAY, 5 DAYS PER WEEK
- REQUIRES TWO THERAPIES BASED ON INTERDISCIPLINARY TEAM APPROACH
- DEMONSTRATES MEDICAL STABILITY
- RETAINS SUFFICIENT MENTAL ALERTNESS TO PARTICIPATE IN PROGRAM
- IS CAPABLE OF DEMONSTRATING SIGNIFICANT PRACTICAL IMPROVEMENT IN A REASONABLE AMOUNT OF TIME
- HAS A VIABLE DISCHARGE DESTINATION
PHYSICIAN'S WRITTEN ORDER SHOULD INCLUDE THE FOLLOWING:
- PATIENT'S NAME
- DIAGNOSIS
- ROOM # AT CURRENT FACILITY
- NAME OF REFERRING PHYSICIAN
QUALIFYING DIAGNOSIS for Inpatient Rehabilitation
- Stroke
- Spinal Cord Injury
- Congenital Deformity
- Amputation
- Major Multiple Trauma
- Hip Fracture
- Brain Injury
- Neurological Disorders
- Burn
- Active, Polyarticular Rheumatoid Arthritis
- Psoriatic Arthritis
- Seronegative Arthropathies
- Systemic Vasculidities with Joint
- Inflammation
- Severe or Advanced Osteoarthritis with proper documentation
- Knee or hip replacement, if criteria met.
- BMI>50 or > 85 YEARS OLD.
Upon receipt of the referral the admission process will begin. These steps will include:
- Onsite evaluation of patient at the referring facility by LIFE Center staff.
- Approval for admission by LIFE Center Medical Director and or admitting physician if different from medical director.
- Notification by LIFE Center Admission Department to referring facility of approval and bed availability.
- Completion of Memorandum of Transfer if patient is being transferred from an acute care facility
- Items required for admission from another acute care facility include Face Sheet, Discharge Order, Current MAR, H&P, OP Reports, Labs, and X ray results.
