Admission Criteria
For Skilled Nursing Unit

A patient in need of SKILLED NURSING CARE must meet the following admission criteria:
REQUIREMENTS for Admission:
- THE PATIENT'S DOCTOR CERTIFIES THAT THE PATIENT NEEDS SKILLED NURSING, WITH A WRITTEN ORDER.
- THE PATIENT HAS BEEN IN THE HOSPITAL FOR AT LEAST THREE CONSECUTIVE DAYS, NOT INCLUDING THE DAY OF DISCHARGE.
- THE PATIENT MAY BE ADMITTED TO SKILLED NURSING UNIT WITHIN 30 DAYS AFTER BEING DISCHARGED FROM AN ACUTE HOSPITAL STAY.
A patient in need of skilled nursing requires one or more of the following services daily:
- Nursing evaluation and management
- Nursing observation and assessment
- Nursing and or rehabilitative teaching and training
- Direct skilled nursing services, IV Medication, injections or wound care
- Physical Therapy, Occupational Therapy or Speech Therapy services
ADMISSION PROCESS TO THE LIFE CENTER SKILLED NURSING UNIT
Admission referrals may be made by calling 903-870-7265. You may also call the main switchboard and have the operator page admissions at 903-870-7000.
Please fax following referral information to the Admission Office: 903-870-7312
Please include the following:
- A written order from referring physician to the LIFE Center Specialty Hospital Skilled Unit.
- A copy of the Patient's face sheet.
- The Patient's diagnosis.
- The Patient's room number.
- The Patient's history, physical and lab results.
- The Patient's current MARS.
- The patient's PT/OT/ST evaluation and notes.
Once the patient's referral has been received, the admission process will begin.
- Skilled supervisor will review patient information and approve or deny admission.
- Admission Department will notify Facility of acceptance or denial.
- Upon acceptance of referral, fax Discharge Summary and Updated MARS.
- Patient transportation will be arranged.
- Report should be called to 903-870-7550.
