Patient Responsibilities
Confidentiality
- You have the right to review the information contained in your medical record. If your doctor says you shouldn't see a part of your record, you have the right at your expense to have another doctor of your choice review that decision.
- You have the right to confidential treatment of your personal and medical records. Information from these sources will not be released without your prior consent, except in your transfer to another health care facility, or as required by law, or under third party payment contracts.
- You have the right to be informed of the current and future use of products of special observation and audiovisual techniques, such as one-way vision mirrors, tape recorders, television, movies or photographs.
- You have the right to be informed of the purpose of the patient assessment data collection.
- You have the right to have any patient assessment information that is collected remain confidential and secure.
- You have the right to be informed that the patient assessment information will not be disclosed to others except for legitimate purposes allowed by the Federal Privacy Act and Federal and State regulations.
- You have the right to refuse to answer patient assessment data questions.
- You have the right to see, review, and request changes on the patient assessment instrument.
