Your Responsibilities

In addition to the preceding legal and human rights, you must also accept certain responsibilities in order to receive maximum benefit from the LIFE CENTER Program.

1. You will be responsible for providing complete and accurate information about your health and for reporting effects of treatment.
2. You will be responsible for participating in the development of your treatment plan.
3. You will be responsible for attending therapy sessions and participating in activities prescribed by the treatment plan you approved.
4. You will be responsible for consideration of the rights of other patients while in the LIFE CENTER Program.
